I am running AstLinux 0.4.3, Asterisk 1.2.10.  (AstLinux 0.4.4 is the  
latest)

Looks like you have an old version of AstLinux?

Also, notice that my example extensions.conf got wrapped, there  
should NOT be a space in "${CALLERID(num)}".

A simple "extensions reload" at the asterisk command line is all that  
is needed.

Did you move your email_in.sh file to "/mnt/kd/", I just used that as  
my example.
