/etc/misdn-init.conf should be a symlink. Here is one of my boxes:

ingrampbx ~ # ls -l /etc | grep misdn-init.conf$
lrwxrwxrwx    1 root     root           24 Apr  8 11:59 misdn-init.conf ->
/tmp/etc/misdn-init.conf

If it is not, recreate the symlink (making sure you save any config which
you might have put into /etc/misdn-init.conf first). /mnt/kd/misdn.init.conf
should then be used automatically.

See this section on the wiki about how all these files and links work (this
documentation is specific to Asterisk configuration, but holds true for
other files as well - documenters, I think we need to clarify this once
more, as there is still uncertainty):
