Look at the sip.conf comments for compete descriptions of "externaddr" and "localnet", but "localnet" should encompass all your local networks (ie. no-NAT) and "externaddr" is your Public IPv4 address on your pfSense external interface, which hopefully is static. BTW, if you do not require external public SIP devices registering to your box, then "externaddr" is not an issue, in that case your only inbound SIP is via a SIP trunk you register to which will track your real public IP address automatically. There are *a lot* of settings you need to add into sip.conf . Each local phone has an entry as well an any SIP trunks.
