If I understand you correctly, your AstLinux box sits behind some other router 
doing NAT.

Normally this should work by default, when your Asterisk outbound peer 
re-registers the new network path should be updated.  Make sure your edge 
router does not have any SIP ALG stuff enabled.

Having the AstLinux firewall enabled should not be a problem, since Asterisk 
will not be NAT'ed by AstLinux, only your upstream router.

You might try enabling "res_stun_monitor" via res_stun_monitor.conf .  You will 
need an appropriate public STUN server.

Asterisk 11 and 13 have ICE/STUN/TURN support in sip.conf using
--
icesupport = yes
--

Personally, my AstLinux boxes are the edge router so this is not an issue.
