The PhoneSuite CTI client works fine, but as the name says it includes much more. You also can use their cheap Asterisk TAPI connector for outgoing-dialing-only from Outlook. Both have usually a 30-day-trial period. Regarding the Snom phonebook sync I am not sure how to do that, but I know that the Snom phones support LDAP and AstLinux includes an OpenLDAP server.
