        "Unusual" is a relative term... I read over 10 million feeds every
day, many of them multiple times each day. I intend to read every public
feed that gets created. Thus, I'll be reading at least 100 million feeds by
the end of this year. I currently maintain several hundred thousand Atom
feeds. That will grow to millions. Thus, I will see every one of those
"unusual cases." 
        What may be rare to the vast majority of participants in this
conversation, who only maintain one or a handful of feeds, will not be
"unusual" to me...
        Can you tell me, in those "unusual cases" when there is "difficulty
in determining which instance came last," what the heck am I supposed to do
if the users expect to always see the "most recent" instance?
