I'm late to answer, but I've found the lag or delay does not happen for me (using Jaws( if I perform actions continually one after the other, i.e. pressing a key directly after another. if you don't want to button mash, press and hold shift an instant before performing an action to cause the lag to be nearly eliminated for the secondt key stroke, because holding down shift counts as a key stroke being continually pressed, and since shift most times doesn't perform an action, it won't cause an action other than the second key you press. For example, when you are in your inventory and you want to move down through your items, you can press and hold shift an instant before you press the down arrow to eliminate the lag from pressing the down arrow for the first time. Keep in mind however that once you stop pressing the arrow key for longer than half a second or so, the next time you press the arrow key the lag may return for the next key stroke, as holding shift isn't perfect. If done correctly, this method makes game play much easier.
I don't know if this works for other screen readers, but it can't hurt to try.
