For getting ASAP to speak in DOSBox, the mainline official dosbox builds don't work very well with dos screen readers. I raised this with the devs, but they're only interested to do the bare minimum to get it to play games so emulation of certain parts of the system isn't as accurate. However, there are projects that fill in those gaps and they make dos screen readers work pretty well. The one I bundled with Talking Dosbox is Dosbox SVN Daum, but Dosbox- also works well.
