I know only that Unspoken does not lag, but as for why people use this, if it's efficient, it is much faster hearing a quick sound than button, checkbox, link and so on. You hear it at the same time as speech, so while it's reading a control, you already know what it is. In fact, in familiar contexts and situations, you can accomplish a lot even without listening to speech and just based on the sounds.
