The HF roll-off thing is interesting - it seems that a flat response is
"too bright" for most folks - even those (like me) who can't hear much
above 15K.

Headphones are used to check for faults - not for balance mixing or
mastering. Usually large "studio" monitors are supplemented by a
variety of smaller (eg nearfield) monitors and various other devices to
create an average balance. However,  seasoned sound engineers/producers
will have an instinctive knowledge of how the mix will sound based on
the baseline reference of a given studio monitoring system - it's like
they can mentally "imagine" what will sound good on their model of an
average system. This is a great skill.

Sometimes (not very often) they get it wrong. Sometimes the mastering
lets them down (again - rare these days).
