Only when you fail to recognise that the spin has turned in to a spiral
dive and overstress the fuselage so it buckles during the "recovery" thus
losing control of the aircraft and other bits that fall off during the high
speed dive prior to ground impact.

None of which would necessarily be fatal if you wore parachutes, started
this at 6000 feet AGL and had a plan to abandon ship at 3000 feet AGL if it
was still spinning after taking recovery action or bits fell off etc.(like
canopies on certain types - hasn't lead to a complete canopy departure/bash
the tail off yet AFAIK but it remains a possibility).
However doing this would be regarded as too difficult just to fulfil the
"annual spin check requirement" so those in charge are happy for you to
risk your life unnecessarily  by not taking these sensible precautions.
