Starting to think like a Coroner, but so are others!

A Coroner will, after establishing the cause of death, make recommendations to 
reduce or negate recurrence of the cause of death.

The Coroner will consider such things,amongst others, as the foreseeability of 
the risk and, within practicalities, it's elimination.

It has been nearly 50 years since Fred Hoinville lost his head in a low powered 
shallow trajectory launch.

We've learnt a lot, but obviously not enough about eliminating the terrain 
risks related to a low trajectory launch profile.
