It seems to work well and non-technical end-users can create simple forms.
 If you want to pull data from LDAP / Active Directory, database, web
service, etc. obviously some technical ability is required to set up the
"data source".  It can email form submissions and also create the back-end
database tables required to save the form if need be.  Forms can be
anonymous or authenticated - recognising the current user and pre-populating
form details with data from AD, etc.

I've not used it to create forms myself, however, I have used it as an
end-user to complete forms.  Check out their fact sheet and/or demo.

On the down side, they appear to be a bit of a tin pot outfit based on a
system I've been asked to look at for performance problems (for which the
reason is obvious to anyone with a clue about relational databases, XML and
web services).
