Hi Jean

I've put Ch11 back into the feedback folder but as it's my first effort at
doing indexing I think someone should double check it.

Are there any more chapters to do because they all look as though they've
been done, so thought I'd double check.  Sorry I've been so slow, I think
I've caught a bug.  I've felt rough all day plus it's been really hot (for
me anyway - it's probably only a mere 25C to those of you who can stomach
the heat lol )
