Starting in chapter 3, images are missing in your PDF. Figs 46, 50, 52,
53, 55, 56... and many more throughout the book. They appear to be the
same ones that are missing in the PDF I created from the same files on
my machine, but I have not checked all the way to the end of the book.
We may have the start of a bug report here, with a bit more testing to
see if it's reproducible on other operating systems.
Later today I will look at the incorrect x-refs and see if they are the
same in both PDFs. Now I wish I had kept a complete list of the ones I
fixed yesterday, to see if any of them have returned.

I have downloaded your zip file and currently have the master doc open
on my WV 3.2.1 system.
At least two figure refs in Chapter 2 are wrong: they still have their
original figure numbers (from the .odt files), but those numbers are now
assigned to figures in Chapter 1, and that's where the refs go to. On
the first page of Chapter 3, the refs to "Setting a default template"
and "Examples of style use" go to the correct text, but the page numbers
displayed are wrong (too high by about 5).
I'll try a PDF export (caution: never did one of those) and check the
figures you mentioned.
This ref problem is definitely worth a bug report, and thanks to your
work, I have all I need to file one. Will do, after trying to simplify
the files.
