I am at work at present so just a quick response.
If you set up a Base system with a spreadsheet as the data source you can
use the existing information on creating a form etc.
If you want to do it all from within Calc,

Set up your spreadsheet with entries for column names, and if appropriate
some information in the first column (probably not necessary, but one
complete line is not bad to have for testing).
Highlight the range of labels and the dummy record, Data->Define Range.
Then Data->Form should open a form with your field names and an entry
area. Step through any data you have with Last Record/Next Record. Add new
data by clicking New and entering fresh data.
You can see it immediately in your spreadsheet.
