[authors] Spreadsheet on status and authors of chapters

Here's the spreadsheet I put together to try to keep track of the status 
and authors of chapters of the user guides. I think it's up to date as of 
yesterday.

You'll notice a conspicuous lack of information about the Database Guide in 
particular. I'll keep filling this in as I collect information, and as 
things change (eg chapters get published).

Anyone else is also welcome to update the spreadsheet, for example when you 
start writing a new chapter, or you revise the outline for a book. I'd 
particularly like to have someone from the DB guide fill in that page!
