The Printing chapter was moved up as suggested, Appendix A was removed

I've re-read what I said in an earlier note and I see that I didn't make
clear what I meant. I said,
"That [Migration Guide] chapter could also be an appendix in the Calc
Guide, but I would think that a reference to the Migration Guide would
probably be sufficient."
What I meant by "a reference to the Migration Guide" was to put an
explicit reference to the Migration Guide somewhere else in the Calc
Guide (not sure where; haven't thought about that detail). Having an
Appendix with only a ref to somewhere else seems a bit silly to me.

Okay, I wasn't sure what you meant by a reference. Maybe in the intro
chapter or something like that?
