[authors] PING All : Using Forms in Writer back up for review

Using forms with Writer is back on the review list.  Thanks for your 
suggestions Sigrid, most of which I've accepted.  I've also added the 
missing material with just one thing I haven't figured out (how the 
Automatic Control Focus button works).
