Ok, I've figured it out. As far as I can tell, it's not a bug.
It's something that went wrong with our "template" file, probably
when I was doing something else. Obviously I didn't catch this
before.
The problem is that in our template, somewhere along the way the
Outline Numbering got the wrong paragraph style listed as the
Level 1 style, and it was showing the style associated with the
"Contents" heading. The "chapter name" field used in the page
header picks up the first paragraph with the style listed as
Level 1 in the outline numbering.
I'm now going through all the chapters checking to see which ones
have this wrong, and fixing it.
