A few additions or changes, several notes, a couple comments.
I am swamped with work and under doctor's orders to get away from the keyboard a lot more. Also, I have worked on that chapter so much in the past that I really can't see it any more.
If you had the time and interest to draft a revised version incorporating your suggestions for reorganisation and other changes (in addition to the changes you have already made), that would be extremely helpful. Otherwise, if it's left to me, I fear that the chances of anything non-trivial getting done are rather low.
