Usually I accept/reject one set of changes before returning a file to
the review list, but sometimes I (or whoever submits the file) forget
to do so. Occasionally we leave older edits in for various reasons.
This one was probably in the "I forgot" category.

Edit > Changes > Record and > Show are both still on.  (When I did this, I see lots of older 
editing.  Is this normal?  I rarely use the Edit > Changes functions on a file that someone else 
has already edited, so I am not sure what is "normal").
