I should have added: whatever workflow we set up must be practical -- it must actually work, given our small team of active volunteers.
For example, part of our current workflow is that each book has an editor whose responsibility includes publishing chapters when they were ready. IMO this is an excellent plan. However, all along we've had too few people who have been both willing and able to fulfill this role. So we've had the choice of someone (usually me) publishing stuff without sufficient review, or chapters not getting published at all.
