Thank you *very much* for writing this document. I have not had time to
read it in detail, but I have also been looking for some instructional
information on XForms for a long time and, like you, I have found very
little.
One of the few articles I did find is this file on J. David Eisenberg’s

Can I get author permission?
As others have noted, you need to register at the OOoAuthors website
(not just for this mailing list) and let us know your login name before
we can give you author permission.
