Kirk has made major organizational changes, and added new material, to
the wiki version of this chapter. I have now incorporated his changes
into the ODT file and uploaded it to the CG3_draft folder. This
chapter still needs editing and further revision and review, including
consideration and incorporation of BMT's review comments (see below).

I have also annotated a file containing BMT's review comments on an
earlier version of this chapter. Many of her comments and questions
are unanswered (and have not been incorporated into the latest
version) and therefore need to be considered by whoever next works on
