Kinda strange, it's always the "start debugging" command that fails,
i.e. the target doesn't answer the JTAG initialization.

Curious, is the board's JTAG known to work well?  I.e., could you
attach a JTAGICEmkII to the same board, and it works?  You might also
give the -R (aka. --reset-srst) option a try, to force an external
reset through nSRST when attaching.  This can e.g. help in situations
where the target is asleep by the time you try connecting.

Ahem, I have not implemented automatically setting the OCDEN fuse
before starting the debugging session.  That's not perchance your
problem?  Use AVRDUDE to set this fuse first, please (by now).
