According to xinit's manpage, it uses ~/.xinitrc by default. That's not really 
qingy's fault I guess.. I suggest you source the system-wide config from there 
(place it in /etc/skel if needed for new users).

That's hard to answer without looking at your xinit config ;)
It could just be dbus-launch thinking your session has ended for some
reason.

I maintain the windowmanagers for Source Mage GNU/Linux and have been using 
qingy for years.
So far I had relatively few problems with it.
Source Mage changes very little from the qingy upstream default
configuration so your troubles might have been distro specific ;)
