I'll throw my name in the hat - it happens on 3.3.1 on FreeBSD for me.  My 
laptop is still running
3.3-r4 (port version - uses rc4 tarball) with a similar config and this does 
not happen.  The only
thing different between them (besides the version) is that my desktop has 
multiple monitors.

I haven't seen anything at all in stdout when this happens, either.  It is 
fairly random with the
exception of only affecting xterms.
