NB. Treats the 'input' action as the output when !serverSide in the

       Codegen for services.xml to add the actionMapping,
outputActionMapping and faultActionMapping elements,

fault, currently picks first one. (wanted to submit the patch as it is
currently suffering daily merge conflicts)

Support setting the correct wsa:Action on response messages
These Action values can some from 3 places:
     1. services.xml (actionMapping, outputActionMapping and

faultActionMapping elements - the latter break apart generic multiple
actions to specific in, out, fault)

     2. wsaw:Action value in a WSDL 1.1 file
     3. generated using the DefaultActionPattern from a WSDL 1.1 file
wsaw:Action values added to generated WSDL when present in the

AxisOperation object
