With the out-of-the box Axis2-1.0, I run into the same behaviour. If you put 
log4j.properties in the client's classpath you should get  
"org.apache.axis2.AxisFault: Transport error 500 . Error Message...". I'll 
attach the log4j.properties file, too.

Actually, as you posted out, there is only one response for two requests since 
our second request causes an error. It is clearly seen in sniffer output 
(message_flow.txt).
