EndpointReference serialization/deserialization should be consolidated in the

Currently, endpoint reference serialization/deserialization code exists in four
different places: EndpointReference, AddressingFinalInHandler,
AddressingSubmissionInHandler, and AddressingOutHandler. This code should be
consolidated in one place, EndpointReference. This would greatly simplify the
addressing handlers. Patch to follow.
