Conform to ASF Source Header and Copyright Notice Policy

The source header and copyright notice policy [1] gives direction on the
content on the source header. Importantly it shouldn't contain a copyright
notice, which should be moved to the NOTICE file. I just did a quick scan of
kernel (which I guess would be representative of the whole) and almost all have
a copyright notice in the source header. We should change this for the next
release to conform to the ASF policy.
