JAXWS: MTOM attachment is lost if outbound JAXWS handler is installed

Problem Description:
The JAXWS message's internal form is converted from OM to SAAJ when a JAXWS
SOAP handler is called. After the SOAP handler chain completes, the SAAJ
message is converted back into OM.
Currently, if the OM contains an MTOM attachment, the attachment is lost during
the SAAJ -> OM conversion.
Solution:
The solution is very simple. The SAAJConverter code is used to convert the
SAAJ message back to OM. In my sandbox, I have modified this code to
optionally accept an OM AttachmentMap.
I also have a added a unit test to verify the code.
I will commit this change after I complete a few more tests.
