This problem is more complex than I originally thought because the WSDL Binding
spec (http://www.w3.org/TR/2006/WD-ws-addr-wsdl-20060216/) requires the old
WSDL instance namespace, so we'll need to support both namespaces. Also if we
want to interoperate with older releases of Axis2 we'll need a way to be able
to use the old namespace with the Metadata spec too.
