Axis2 1.0 supported only inbound messages, since MTOM messages are backward compatible with SwA messages. We expected the users to send MTOM instead of SwA to the SwA endpoints...
Good news is Axis2 1.1 and later releases have full support for SwA.. Now you can invoke a web service with an pure SwA message using Axis2. You can have a look at the Attachments sample which is shipped with Axis2 release.
