I've made a change to how AxKit handles a request, and I'd like a sanity check 
before I just check it in...

What I've done is add a flag that says we're in the fast_handler, and in the 
Provider, only do the $filename =~ /.xml/ or $type =~ /text\/xml/ checks if 
we're *not* using the fast handler.

This means that if you go:

  AddHandler axkit .pod .sxw .xml .axp .xsp

You no longer have to also set all the content types of those files to 
text/xml (since this wouldn't make sense for some of them). I think this is 
becoming one of our most seen FAQ's, and as such it should be considered a 
bug.

I'd also like a check on the passthru behaviour. Taking a .pod file (and the 
new Pod provider written by Kip) as an example, here's what happens when you 
request a .pod file:

 - If passthru=1 but PerlSetVar AxFastPassthru is unset or 0, you get the 
generated XML.

 - If passthru=1 and PerlSetVar AxFastPassthru is set to 1, you get the source 
POD.

 - If passthru is zero or unset, but there are no styles defined, you get the 
source POD (I think this is a bug - comments please).

 - If passthru is zero or unset, and there are styles, the POD is transformed.
