Gah! I've gotten so caught up in other things that I haven't
documented a vital change in that accompanies my patch.
Apache::RequestNotes must be installed as a PerlInitHandler.
Example:
PerlInitHandler Apache::RequestNotes
In testing with the latest A::RN from CPAN, behavior seemed to be
modified significantly so that it didn't work as before and this
change is needed.
I suspect that this while help you solve your problem.
