Carmen, I checked my original email from Marian, who works the the USCIS. She gave the same link you had. She said the form number is G-1041 and unless you already happen to have your immigrant's A number, then you need to request the Index Search.
On that Web site, on the right side, I clicked on "Make A Genealogy Request." It looks like you can order online.
