I can read most of it until the 6th line. (except the mothers name...Antonia Maria???) I can make out the birth date to be 20th of November? 1756 and baptised on the 30th?? at the Ermida da Nossa Senhora da Natividade. It's the rest that is very difficult to read and i'm not sure if there is any important info in there (besides priests name..etc..) Godfather Francisco da Rosa Botelho? and his sister?
