Isn't Jerry a nickname for Gerald?  I can't think of a Portuguese
equivalent at the moment.  But some people do other things.  An aunt was
born Gladys Gwendolyn and went by Peggy.  She didn't like her names.  And
I've had immigrant students with names from their nationality, and they say
they go by "Steven" or "Camille" or some name that they like.

Maybe your answer will be the 1940 census (to be released unindexed in
April).
