I also recommend viagens aquarius for Sao Jorge. I used them when I was there and they were very helpful, even told me about an express ferry to get there from Pico, what time it left, met me when I got to Sao Jorge, took me to my hotel, took me on a tour of the island and arranged for a cab ride to the airport on the day I left. They even helped me try and find the house where my grandmother had lived! Did I mention I don't speak Portuguese? If your relatives do speak it, they won't need as much help as I did.
