I think you should conclude that Andre Rodrigues has two names, Rodrigues or Teixeira, even if you don't see the Teixeira again. Of course it can be a mistake, but if the records permit, I think you will find it more likely that Teixeira is part of Andre's heritage. I have run into too many cases like this. It looks to me like a line from Santa Maria, where Faleira is more common and perhaps originated.
