Paul, the record begins with "Jose, exposto em casa de Manuel da Silva "quebra casas" desta freguesia. It means Jose was left at this family's door. At the bottom it reads the child was handed over to Manuel's wife Domingas to be raised. It sounds like this may have been a one time thing for this couple of that she in fact worked for City Hall by raising abandoned children. My conclusion would be the latter thus the reason someone knew that and left the child there.
Another thing to note is the "nickname" quebra casas (house breaker) could mean he was a house demolisher if that was his trade or it may have other nuances related to his personality. Regarding adding the child in my opinion, this not being an adopted child I would not add it to the family tree but would make a note of it. Hopefully others in the group will give you their input also.
