I'm not completely sure but I think it is saying a certificate of this record was given on that date.
Specifically: 1.a C.am (an abbreviation for primeira certidão) as 10 de Dezembro 1869. O Cura Oliv.a (Priest's name Oliveira) He might have needed a certificate for a marriage or a passport or something else.
