Liz, What you do with the blank forms....Fill out part of it for him. But make sure you put an obvious mistake. If he was born in 1937, make him born in 1927. Or 1973. Don't make the form full of mistakes, but make one he would catch, such as his birth date. Enclose a self-addressed, stamped envelope. Trust me, you make a mistake on his birth date, and he'll correct it and send the form back to you.
