I have recently used the above website to obtain birth records. You need to
have your Portuguese ID number and create an account.  The site in my
opinion is not user friendly.  Through some trial and tribulation I have
managed to use it successfully. They charge 20 euros per certified copy.
You need to have enough information to fill out the required fields.  It
then takes 2 to 3 weeks to receive in the mail. Credit cards accepted as
payment. Please note that depending on your credit card you may be charged
additional foreign transaction fees.
