After the godfather, which as Sam said is Antonio Homem da Costa (who is 
single), it is mostly the normal church script as Cheri said.  I think the 
confusing part is that they did not name a godmother.  Instead it says that 
for Madrinha they invoked Nossa Senhora touching the crown of the statue 
and it names Francisco Pereira Cachete (a married stonemason).  I assume he 
must the one that did the invocation or is named as a witness to it.  I am 
not familiar with this practice.  

My translation might not be exact but I hope it gets the gist of it.
