This was happening to me when I was using rsyncd and File::RsyncP on the server that ran protocol version 26. Upgrading it to run protocol 28 with CPAN fixed my problem. You said ssh+rsync, not rsyncd tunneled through SSH right? So maybe this doesn't apply to you.
