I can believe that the changes you made resolved the problem, because a number 
of the problems seem to be a result of several things happening at the same 
time.

However, the root of the problem is a "race condition" (a bug) in the Bacula 
SD.   Hopefully, the next version of Bacula will fix this.    If I get a good 
dump showing a race condition, we can probably send a patch, but without 
knowing exactly what is going on (i.e. a good dump) there isn't much we can 
do.
