I am now 90% convinced that this problem is related to over driving the SCSI or the tape drive. I believe that I have a fix for it in 1.39.20, which was recently released into Beta. If you are still able to produce this problems, I would appreciate it if you could test the beta version to see if the error (OS or driver error, IMO) still exists.
